Legal question, read on.....power of attorney and kids...? - medical power of attorney for kids

I am planning a trip Sugust I have 3 children ... 19, 16 and 15, want to prove that 19 years is to make the power and the ability to medical decisions for their brothers ... And if possible, to do the same for 16 years to 19 years.
So my questions are:
In what form (s) should I use?
Where are they available? possibly free.

Thank you very much .. was surfing the net, and I see 2 ways it could work, just want to be sure.

My father took my two younger sisters gave temp power for a few months while he was moving. Everything he did was an Internet download to show what they could, and what is not. Just do a search for "power was authorized" to make medical decisions, judgments and decisions about their education. He has signed and not noterized and at the same time a start date and completion. Noterized costs nothing, you have it in banks and police stations, that many employees are often notery officer or dispatcher

In most countries in the 19 years as a lawyer acting for you in terms of two young brothers. If you live in the United States or Canada, may appoint counsel for minors, but could not be performed requires a contract, the consent of an adult. There is no point in the distribution of power.
